Services Cancellation Form

Please read before submitting. Services carry the initial term stated in your signed proposal. If you cancel before the end of that term, the Early Termination Fee in Section 13 of the Marketing Services Agreement applies and is charged to the payment method on file. After the initial term, 30 days written notice is required and fees continue through the notice period. Submitting this form does not stop billing for the current period, and does not change any amount already due.
